POWER OF ATTORNEY

This Power of Attorney (the “Power”) is made and entered into between [name of prinl], an individual with a prinl address of [prinl address] (the “Prinl”), and [name of attorney-in-fact], an individual with an attorney-in-fact address of [attorney-in-fact address] (the “Attorney-in-Fact”).

WITNESSETH

WHEREAS, the Prinl desires to appoint the Attorney-in-Fact as his/her true and lawful agent and attorney-in-fact for the purpose of [describe purpose of power of attorney].

NOW THEREFORE, in consideration of the foregoing premises and of the mutual covenants and agreements herein contained, the parties hereby agree as follows:
